Login
American HomePatient will notify you via e-mail that an e-CMN
is awaiting completion.
To
get started, simply access the e-CMN server via the secure
Internet site www.carecert.us,
then log onto the system using your unique user name and password.
This
system far exceeds the paper CMN process regarding confidentiality,
since only staff members who complete CMNs will have access
to the patient’s information! |

Home
e-CMN Home serves as your “inbox” for
e-CMNs awaiting completion and/or signature. This window
provides you with
information at a glance about the waiting e-CMN, including
patient name; type of services to be provided (i.e., oxygen,
nebulizer therapy, etc.); and current status of the e-CMN.
Simply
click on the e-CMN and select "Edit" to begin completing
Section B. |

e-CMN
Cover Letter
The e-CMN is routed to you with Section C already completed
by your local American HomePatient office. For your convenience,
we also include a Cover
Letter that restates the information
we received about your patient upon referral.
Our
Cover Letter appears on a split screen along with the e-CMN.
This split-screen technology allows you to scroll through
the Cover Letter and conveniently reference test scores
and
other relevant patient information as you complete the e-CMN.

Electronic Signature
After Section B of the e-CMN has been completed by your authorized
staff, and all sections have been reviewed for accuracy,
the
document is ready for electronic signature by the referring
physician.
Only
the referring physician can electronically sign the e-CMN
by accessing the document using a secure logon and password.
The physician also may make any necessary changes to sections
A, B, and D. When the document is signed electronically,
the
system will automatically apply the date in Section D and
ratify the physician’s changes. |

You’re done! Automatic processing takes
